The long-term use of pediatric medications can lead to discoloration of dental restorations, which affects their durability and longevity. The present in vitro study evaluated the effects of commonly used pediatric medications on the color stability of a conventional and a bulk-fill composite resin. For this study, 80 disc-shaped specimens of each composite were prepared in molds with a diameter of 6 mm and thicknesses of 2 or 4 mm (n = 40 per thickness per material). A spectrophotometer was used to evaluate the baseline color of the specimens in the International Commission on Illumination L*a*b* color space. Each specimen was immersed separately in a container holding 1 of 8 liquid medications (n = 5 per thickness per medication): amoxicillin/clavulanate, clarithromycin, cephalexin, acetaminophen, ibuprofen, levetiracetam, multivitamin, or albuterol. After the container was shaken for 2 minutes, the specimen was removed from the medication and stored in artificial saliva. The cycle was repeated every 8 hours for 1 week. The color measurements were repeated after 1 week of immersion cycles, and the overall color change (ΔE*) was calculated; a value of ΔE* > 3.3 was considered clinically perceptible. The data were analyzed with 1-way and 2-way analyses of variance as well as the Levene test and Games-Howell post hoc test (P < 0.05). All specimens displayed clinically perceptible color changes after exposure to medications commonly used by children. The mean color change in the 4-mm bulk-fill composite resin group was significantly greater than that in all other groups (P < 0.05). However, there was no significant difference in color change based on the immersion drug for either of the composites (P > 0.05). The study findings show that exposure of composite resin to certain commonly used pediatric drugs causes color changes that are clinically perceptible.

OBJECTIVE: Considering the rising prevalence of metabolic syndrome (MetS), this study aimed to investigate the relationship between MetS and its components with oral and dental health in the adult population of the Azar cohort. METHODS: In this cross-sectional study oral health care behaviors, DMFT (decayed, missing, and filled teeth) index, and demographic data related to 15,006 patients (5112 in the MetS group and 9894 in the healthy group) of the Azar Cohort population aging from 35 to 70 were collected using appropriate questionnaires. The definition of MetS was based on the National Cholesterol Education Program Adult Treatment Panel III (ATP III) criteria. Then, the risk factors of MetS related to oral health behaviors were determined by proper statistical analysis. RESULTS: The majority of MetS patients were female (66%) and uneducated (23%) (P < 0.001). In the MetS group, the DMFT index (22.15 ± 8.89) was significantly (p < 0.001) higher (20.81 ± 8.94) than the no MetS group. Not brushing at all was associated with increased odds of MetS (unadjusted OR = 1.12, adjusted OR = 1.18). Flossing less than once a day was associated with increased odds of abdominal obesity (unadjusted OR = 1.17, 95%CI = 1.03-1.32) and hyperglycemia (unadjusted OR = 1.88, 95%CI = 1.61-2.20). CONCLUSIONS: This study showed that in MetS patients of the Azar cohort study, oral hygiene was worse compared to that in the no MetS group. Further studies are suggested to encourage oral hygiene among the general population which has more beneficiary effects than has been known before.

BACKGROUND: The coronavirus 2019 (COVID-19) pandemic had several mental effects on medical staff. OBJECTIVE: This study intended to evaluate deviations in Frontline Dental Staff (FDS) depression, perceived stress and hypochondria during the COVID-19 outbreak, and to assess whether the existence of potential risk-factors influenced these symptoms over time. METHODS: This prospective cohort study surveyed FDS three periods from February 1, 2021 to April 8, 2022. FDSs' mental health was assessed through Beck Depression Inventory, Cohen Perceived Stress and the Evans Self-Illness Questionnaires; multivariate linear regression were used to identify potential factors associated with changes on each outcome (depression, perceived stress and hypochondria) over time. All analyses were performed in STATA ver. 14. RESULTS: In a total of 635 FDS, about 69%, 83%, and 71% reported relative depression, 77%, 87%, and 64% reported moderate to high stress level, and 60%, 76% and 74% reported moderate hypochondriac from first, second, and third survives, respectively. The mean and standard deviation of the variables increased dramatically at second survive (July 17 to October 21, 2021) and decreased again at third survive significantly. Being female, working in public sector, and low socioeconomic level were main predictors of higher levels of depression (p < 0.001) and perceived stress (p < 0.001). Perceived stress was a strong predictor of high-level depression (t  =  7.79, p < 0.001). CONCLUSION: FDSs' depression, stress and hypochondria were high and positively associated with being female, working in public-sector, and low socioeconomic level. To increase the effectiveness and decrease work burnout, psychological support should be provided.

BACKGROUND: One of the drawbacks of tooth-colored dental restorations is their discoloration over time. The present study aimed to determine the effect of four categories of pediatric medications, including analgesics, antibiotics, anticonvulsants, and multivitamins, on two types of tooth-colored dental materials, namely, composite resins and glass ionomer cements. MATERIALS AND METHODS: In this in vitro study, a total of 40 specimens with disc shapes (with a diameter of 5 mm and thickness of 2 mm) were prepared from each material and immersed in eight different drugs for 2 min three times a day for 1 week. The values of the baseline color were calculated based on the CIE (International Commission on Illumination) L*a*b* system. After 7 days, ΔE values were calculated. Two-way analysis of variance was employed for statistical analysis. Statistical significance was defined at 0.05. RESULTS: Statistical analysis showed that there were significant differences between ΔE and different restorative materials as well as ΔE and drug types (P < 0.001). CONCLUSION: According to the results of this in vitro study, all the four types of drugs caused the discoloration in all the restorative materials, and the color change values were affected by the type of used drug and restorative material.

BACKGROUND: Various iron drops are prescribed to children to prevent iron-deficiency anemia. The current study aimed to evaluate and compare the physicochemical profile of iron drops and the effect of these medicines on the color of primary teeth. MATERIALS AND METHODS: On the commencement of this experimental study, the pH and viscosity of five types of iron drops were measured. For the purpose of the current study, sixty healthy anterior deciduous teeth were provided; thereafter, they were assigned to five groups and then exposed to iron drops. The color and color difference of each specimen were measured by VITA Easyshade Compact after 2 weeks. The amount of absorbed iron was determined by atomic absorption. The data were analyzed using two-way ANOVA and Tukey's test (P < 0.05). RESULTS: As evidenced by the obtained results, all medications demonstrated acidic pH and discoloration. The viscosity values of iron drops were reported to be within the range of 2.07-33.58 cP. Based on the results of Pearson's correlation coefficient test, discoloration showed a correlation with pH and atomic absorption (P < 0.05). CONCLUSION: Analyzed iron drops displayed low pH and discoloration higher than 3.3 which can be easily distinguished with naked eyes. Medicine labels should include warning statements on the feasibility of dental discoloration and erosion.

Background . Mouthwashes are widely used as adjuncts to mechanical oral hygiene procedures. Nonetheless, there is little information regarding the effect of various mouthwashes on the amount of ions released from the nickel-chromium (Ni‒Cr) alloys used in the fabrication of fixed prostheses. Therefore, the present study was conducted to evaluate the effect of two types of mouthwash on the release of Ni ions from dental alloys. Methods . Forty-two disk-shaped specimens were prepared with a diameter of 10 mm and a height of 2 mm. Two mouthwashes were examined in this study: Oral B and Listerine. A control group was also considered using distilled water. Each Ni‒Cr disk was immersed in the mouthwashes and distilled water in polypropylene test tubes, and then incubated at 37°C to simulate the oral temperature. After 45 days of incubation, the samples were tested for Ni ions using inductively coupled plasma mass spectroscopy. Data were analyzed using ANOVA. Results . In the Halita group subjects exhibited 2.04±0.65 reduction in OLS. OLS reduction in the chlorhexidine group was 1.95±0.74. Statistical analysis showed no significant difference between the two groups (P>0.05). Conclusion . As the results indicated, the amount of ion release was within the safe limits in the two experimental groups. However, it is recommended that prescribe Listerine mouthwash should not be prescribed for the patients with a history of Ni allergy.

Objective: To evaluate the dimensional changes in three types of alginate following three different delay times after casting. Material and Methods: In this laboratory study, a maxillary arch-shaped stainless steel model was prepared and three pins with similar and determined diameters and heights were placed (one at mid-line and two on the underside of the model) as reference points to compare the dimensional stability of the three types of alginates. A special metal tray was made from the main model. The main moldel was imprisoned by the metal tray, and these were kept for a specific time in a humid environment. Gypsum Type 4 was poured over the impression to obtain 90 gypsum casts for three types of alginate. The dimensions of casts obtained from each alginate were compared in two lateral and anterior-posterior dimensions, they were compared with each other, and the alginates were also compared to the main model. Results: At a time interval of 15 minutes, the dimensional accuracy of the casts obtained from the three types of alginates, with the main model in both anterior-posterior and transverse dimensions was not significantly different (p<0.05); but there was a significant difference at 60 minutes and 24 hours in both dimensions (p<0.05). Although there was no significant difference between the alginates in both dimensions and all three studied time periods, the least difference with the main model was related to Zhermack and the highest difference was related to Golchai alginates. Pairwise comparisons showed that none of the samples had a significant difference in terms of dimensions. Conclusion: The dimensional stability of alginate was a time-dependent type of alginate and had no significant effect on the dimensional accuracy of casts.

BACKGROUND: Iron supplement is recommended for alleviating iron deficiency. Black discoloration of teeth is one of the side effects of consumption of iron supplements. Besides these effects, iron drops may also act directly on the dental hard tissues, with erosive lesions as possible outcomes of their content. In this regard, we decided to compare iron absorption of five types of iron drops in enamel and determine its relationship with pH and titrable acidity (TA) of them. MATERIALS AND METHODS: In this in-vitro study The pH of 5 kinds of iron was measured by a digital pH meter. Each drop was titrated and then the TA was calculated. For this experimental study, 40 healthy anterior primary teeth were prepared and divided into 5 groups then exposed to iron drops. The level of iron absorption was determined by atomic absorption. The Pearson correlation test was used to analysis correlations between pH, TA, and absorption of iron ions in the anterior primary teeth. One-way ANOVA was used for statistical analysis. Statistical significance was set at P < 0.05. RESULTS: The mean of pH in Kharazmi drops was significantly less than both Irovit, and FerroKids drops (P < 0.05) but did not differ significantly with other drops. Kharazmi drops TA was higher than all drops (P < 0.05). There was no significant difference between the mean iron ion adsorption, as well as between iron ion absorption with pH and TA also not found a statistically significant relationship. CONCLUSION: All drops have acidic content that increases their potential for erosion. Reducing the potential of the effects of iron drops on dental hard tissues should be of concern to all health professionals.

OBJECTIVE: Different fluoride varnishes are used for the prevention of dental caries. The aim of this study was to compare the antimicrobial effect of different fluoride varnishes, containing xylitol and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), on Streptococcus mutans. MATERIALS AND METHODS: In this in vitro study, the antibacterial effects of four varnishes, namely Polimo and V-varnish (containing xylitol), MI varnish (containing CPP-ACP), and Preventa, were evaluated against S. mutans. The disc diffusion method was used for testing the bacterial sensitivity. The data were analyzed using the Kruskal-Wallis and Mann-Whitney U-tests. RESULTS: According to the results, Polimo showed the highest antibacterial effects, compared to the other three varnishes (P ≤ 0.05). Growth inhibition zones were not observed in V-varnish and Preventa. The mean diameter of inhibition zone around the MI varnish was significantly higher, compared to those of the V varnish and Preventa (P ≤ 0.05). CONCLUSION: As the findings indicated, the fluoride varnish containing xylitol and CPP-ACP could be more effective iAQ1n the prevention of dental caries.

Tooth transposition, which is a rare condition, is ascribed to the disturbance of tooth eruption and the subsequent abnormal occlusal relationships. Transpositions mostly involve the upper jaw and more frequently occur between the maxillary canine and first premolar. Herein, we present a case of a maxillary canine‒second molar transposition in a thalassemic patient.

Tooth avulsion is defined as the complete displacement of the tooth out of its alveolar socket. The treatment of choice is immediate replantation or if that is not possible, placement of the tooth in an appropriate storage media. This report presents replantation of an avulsed maxillary central incisor after 12 hours of storage in milk. The tooth was replanted and splinted. One week later, it was treated endodontically and calcium hydroxide dressing was placed for 1 month; subsequently, the tooth was obturated with gutta-percha. During three years of follow-up, no evidence of ankylosis or inflammatory resorption was observed. After three years, the tooth was stable and remained functional and esthetically acceptable.

RESUMO

Ectodermal dysplasia is a hereditary disorder associated with abnormal development of embryonic ectodermally-derived organs including teeth, nails, hair and sweat glands. Hypodontia of the primary and permanent dentition is the most com-mon oral finding. Therefore, affected patients need dental prosthetic treatments during their developmental years. This re-port presents two cases of children affected by ectodermal dysplasia with anodontia. Oral rehabilitation was accomplished with removable acrylic prostheses. Treatment had major impacts on self-esteem, masticatory function, speech and facial esthetic.

BACKGROUND AND AIMS: The clinical management of oral lichen planus poses considerable difficulties to the clinician. In recent years, researchers have focused on the presence of pathogenic microorganisms such as Candida albicans in the patients with refractory lichen planus. The aim of the present study was to investigate the prevalence of candida species in the erosive oral lichen planus lesions. MATERIALS AND METHODS: Twenty-one patients with erosive oral lichen planus and twenty-one healthy individuals aged 18-60 were randomly selected; samples were taken from the tongue, saliva and buccal mucosa with swab friction. Theses samples were sent to the laboratory for determining the presence of candida species in cultures and direct examination method. RESULTS: No significant difference was found between healthy individuals and patients with erosive lichen planus regard-ing presence of candida species. The type of candida in the evaluated samples was Candida albicans in both healthy and patient groups. CONCLUSION: According to the results, candida was not confirmed as an etiologic factor for erosive lichen planus lesions.
